Credential Applications

Credential applications are now submitted online, after CSULB Recommendation. To request a credential recommendation, download, print and complete the appropriate form below AFTER you have finished all requirements. Do not request recommendation before you have completed required classes, fieldwork, and degrees, and passed all required examinations.  Be SURE that your grades and degree are on your transcript before you print and submit a copy to the Credential Center.

Summer 2008 Credential Application Packets:

• Summer 2008 Multiple Subject & Single Subject Credential Packet (Request for Recommendation)

Important: You must submit a copy of your last semester grades with the Request for Recommendation. Courses must show a grade. If no grade is listed, your recommendation cannot be processed. If you are completing a Bachelor's degree this semester, wait until your degree is posted on your "My CSULB" unofficial transcript; submit a copy of the unofficial transcript showing the posted degree.

Single Subject credentials will be recommended after we receive final clearance from the Single Subject Program Office.

• Education Specialist Credential Packet (Request for Recommendation)

Important: You must submit a copy of your last semester grades with the Request for Recommendation. Courses must show a grade. If no grade is listed, your recommendation cannot be processed. If you are completing a Bachelor's degree this semester, wait until your degree is posted on your "My CSULB" unofficial transcript; submit a copy of the unofficial transcript showing the posted degree.

• Specialist & Service packet (Request for Recommendation)

Important: You must submit a copy of your last semester grades with the Request for Recommendation. Courses must show a grade. If no grade is listed, your recommendation cannot be processed. If a Master's degree is required for your credential, submit an unofficial transcript verifying the Master's degree.
