Rotation Check Out

Upon completion of your SERVE rotation please:

• Check-out
• Turn in Service Hours Verification Form and CSULB Final Evaluation Form to your University Instructor (original) and to the SERVE Office (yellow copy).
• Turn in Reflection Journal to the SERVE Office if you are checking out for an Independent Rotation.
