Confidentiality

Confidentiality is your security that the information you share with your clincian is safe and secure.  While you are free to disclose your personal information to whomever you wish, confidentiality provides you the security that we will not share your personal information with others.  There are exceptions to this, however.

If you indicate that you are planning on harming yourself or someone else, in any way, this information will not remain confidential, as we are legally mandated to ensure that you are safe.

Likewise, if a minor reports that they have been harmed in some way, or report they may be harmed, we are legally mandated to ensure a minor is safe.

How we protect your information: 

At the Community Clinic, we believe that your personal experiences and those documented in your clinical records, deserve the utmost security.  We have a unique training environment, and this requires ongoing dedication to training and practice in confidentiality procedures.  Our Clinic staff, clinicians, and Clinic faculty are trained in confidentiality guidelines.  We have also taken extra steps and measures to ensure that your information remains confidential within our unique atmosphere.

All files are kept under double lock and key, and are available to your clinician, their supervisor, and the Clinic staff that administrate the Clinic, and are ONLY available on an as-needed basis.  While cases are staffed with teams of clinicians, limited identifying information is discussed, and when there may be conflicts of interest, these cases are staffed individually or assigned specifically to avoid conflicts.

All Clinic staff, clinicians, supervisors, and individuals who work at the Community Clinic are required to know and practice professional guidelines related to confidentiality, and our policies specifically state that a violation of these codes would result in loss of practice privileges.

It is important that you do realize, because of our unique training atmosphere, that Clinic staff and Clinic faculty MAY have access to your records in order to effectively train and supervise your clinician. The Director of the Community Clinic may also have access to your records.
